How to complete the questionnaire
①You will be taken to the Medical Questionnaire when you click on "input page" towards the bottom of the page beneath the calendar.

②A QR code will be produced for each questionnaire that has been completed.

③Please make sure to print the displayed QR codes.
*The QR cord is up to three kinds.

The reservation number and the passcode are required in order to check or cancel your reservation.
