＜Important points to note about the online Medical Questionnaire＞

・ Completing the online questionnaire does not record your entries. The information entered is only registered when the 2d codes are read in at the Health Service Center.

・ Make sure that when you print the 2d codes they are in a condition that can be read properly. If your 2d code is disfigured, folded, unclear, etc. it may not be read correctly.
